Questions we hear often.

Straight answers about eligibility, medications, pricing, and what happens at every step.

Care

Meridian Health is a care coordination platform, not a medical practice. Independent licensed physicians and nurse practitioners practice on the network. Meridian does not employ providers and does not practice medicine. Your provider relationship is directly with the licensed clinician assigned to your intake.
All providers are U.S. licensed physicians or nurse practitioners. NPI number, active state licenses, and current malpractice coverage are verified at onboarding and re-verified quarterly. You can view your assigned provider's credentials and NPI in your member portal.
Meridian Health is currently licensed and operational in 47 states. Your state is confirmed at step 7 of the eligibility assessment. If your state is not currently supported, you will have the option to join a waitlist — no charge applies until your state goes live.
Most provider reviews complete within 1 business day of your assessment submission. You will receive an email notification when your review is complete. If additional information is needed, your care coordinator will reach out directly.
Monthly check-ins are async (you complete a short structured update) or sync (scheduled video/phone with your provider, available on Care and Complete tiers). Your provider reviews your progress, adjusts dosage if needed, addresses side effects, and updates your care plan for the next month.

Eligibility

Eligibility is determined by your provider on an individual basis. General considerations include: adults 18 and over, BMI ≥ 27 with at least one weight-related comorbidity (or BMI ≥ 30), no contraindications to GLP-1 therapy (MTC, MEN 2, active pancreatitis), and residence in a supported state. These are guidelines, not guarantees — your provider makes the final clinical determination.
"Provisional eligibility" means your answers to the assessment are consistent with the general criteria for GLP-1 therapy, but your final eligibility is confirmed by a licensed provider who reviews your complete intake. We show this result immediately so you can proceed to membership — knowing that provider review is the final step.
If your provider determines there is no clinical path forward, your onboarding fee is refunded in full. You will receive a notification explaining the decision. You will never be charged a recurring membership fee if a prescription cannot be written. Specific clinical reasons are communicated through your provider, subject to applicable laws.
GLP-1 receptor agonists are FDA-approved for both weight management and Type 2 diabetes. Whether Meridian is appropriate for you depends on your specific history, current medications, and clinical picture — your provider will review this during your intake. Meridian does not treat Type 1 diabetes.

Pricing

The onboarding fee ($197–$297 depending on tier) covers your initial provider intake review, clinical assessment, care plan setup, and onboarding coordination. It is a one-time charge. If no clinical path is established, the full amount is refunded.
On Essentials and Care tiers, medication is billed separately by the pharmacy. Costs vary by medication and dosage. Your provider discusses medication options and estimated cost before the prescription is sent. Complete tier includes GLP-1 medication in the monthly membership — no separate pharmacy bill for the medication itself.
Meridian membership fees are not currently covered by insurance. Some insurance plans may cover GLP-1 medications when prescribed for an indicated condition — your pharmacy can check your coverage when processing the prescription. We recommend checking with your insurer before joining if medication coverage is a factor in your decision.

Safety

GLP-1 receptor agonists are FDA-approved prescription medications with established safety and efficacy data. They are not appropriate for everyone. Contraindications include personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Serious risks include pancreatitis and gallbladder disease. Most side effects (nausea, diarrhea, constipation) are mild and decrease over time. Your provider reviews your complete history before prescribing.
The most common side effects of GLP-1 therapy are gastrointestinal: nausea, diarrhea, vomiting, constipation, and abdominal discomfort. These are most common when starting or increasing dose and typically improve over several weeks. Serious side effects (pancreatitis, gallbladder problems) are rare but require immediate medical attention. Report any concerning symptoms to your care coordinator immediately.
Meridian Health is not an emergency care service. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Meridian telehealth services are for non-emergency chronic care management only. Do not use the member portal to seek emergency care.

Important Safety Information (ISI) — GLP-1 Receptor Agonists

GLP-1 receptor agonists (semaglutide, tirzepatide, liraglutide) are FDA-approved prescription medications. They are not appropriate for everyone. Do not use if you or a family member have a history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).

Serious risks include: pancreatitis, gallbladder problems, kidney problems, hypoglycemia (in patients using insulin or insulin secretagogues), heart rate increase, suicidal ideation and behavior, and serious allergic reactions. Tell your provider about all medications you take and all medical conditions.

The most common side effects are nausea, diarrhea, vomiting, constipation, and stomach pain. These are usually mild and decrease over time.

Not for use in patients with type 1 diabetes. If you experience severe abdominal pain, stop the medication and seek immediate medical attention. For full prescribing information, consult your Meridian Health provider.
