Terms of Service

Last updated: January 1, 2026

1. Agreement to Terms

By accessing or using the Meridian Health website, mobile application, or related services (the "Services"), you agree to be bound by these Terms of Service ("Terms"). If you do not agree, do not use the Services. Meridian Health, Inc. ("Meridian," "we," "our," or "us") reserves the right to modify these Terms at any time with notice.

2. Eligibility

You must be at least 18 years old and reside in a state where Meridian providers are licensed to use our clinical services. Completing the eligibility assessment does not guarantee that a provider will issue a prescription. All prescribing decisions are made independently by licensed providers.

3. Nature of Services

Meridian is a care coordination platform, not a healthcare provider. Clinical services are provided by independent licensed physicians and nurse practitioners ("Providers") who use the Meridian platform. Meridian does not practice medicine or provide medical advice. Meridian is not an employer of the Providers.

The Services include:

  • A platform for completing a health intake and having it reviewed by a licensed provider
  • Coordination of prescription routing to NABP-verified pharmacies
  • Membership-based ongoing care coordination, check-ins, and communications
  • A member portal for accessing care records and communicating with care coordinators

The Services are not a substitute for emergency care, in-person diagnosis, or primary care.

4. Your Responsibilities

  • Provide complete, accurate, and truthful information in your intake and all subsequent communications
  • Notify Meridian or your provider promptly if your health status, medications, or circumstances change materially
  • Use the Services only for lawful purposes and in compliance with these Terms
  • Maintain the security of your account credentials
  • Not share account access with any other person
  • Seek emergency care (call 911) for any medical emergency — do not use Meridian for emergencies

5. Accounts and Security

You are responsible for all activity that occurs under your account. Notify us immediately at care@northmarkhealth.com if you suspect unauthorized access to your account. Meridian is not liable for losses resulting from your failure to maintain account security.

6. Payments

Membership and onboarding fees are described at northmarkhealth.com/pricing. Payment is processed by Stripe. By providing payment information, you authorize Stripe to charge the applicable fees on Meridian's behalf.

Onboarding fees are refundable if the reviewing provider determines there is no clinical path forward for GLP-1 therapy. Membership fees are non-refundable except as described in our Cancellation Policy and Subscription Terms.

Medication costs are separate from membership fees and are billed directly by the pharmacy on Essentials and Care plans. On the Complete plan, medication is included up to a specified quantity per month as described in the plan terms.

7. Prohibited Uses

You may not use the Services to:

  • Provide false or misleading health information
  • Resell, redistribute, or make commercial use of the Services
  • Attempt to gain unauthorized access to any system or account
  • Transmit malware, viruses, or other harmful code
  • Violate any applicable law or regulation
  • Harass, threaten, or harm any provider, coordinator, or other user

8. No Warranties

THE SERVICES ARE PROVIDED "AS IS" AND "AS AVAILABLE" WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED. MERIDIAN DOES NOT WARRANT THAT THE SERVICES WILL BE UNINTERRUPTED, ERROR-FREE, OR FREE FROM SECURITY VULNERABILITIES. MERIDIAN DOES NOT WARRANT ANY PARTICULAR HEALTH OUTCOME OR THAT GLP-1 THERAPY WILL BE PRESCRIBED OR EFFECTIVE FOR YOU.

9. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, MERIDIAN'S TOTAL LIABILITY TO YOU FOR ANY CLAIMS ARISING OUT OF OR RELATED TO THE SERVICES SHALL NOT EXCEED THE AMOUNT YOU PAID TO MERIDIAN IN THE 12 MONTHS PRECEDING THE CLAIM. MERIDIAN IS NOT LIABLE FOR INDIRECT, INCIDENTAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES.

10. Indemnification

You agree to indemnify and hold harmless Meridian, its officers, directors, employees, and agents from any claims, damages, or expenses arising from your use of the Services, your violation of these Terms, or your provision of false or inaccurate health information.

11. Dispute Resolution

Any dispute arising from these Terms or the Services shall be resolved by binding arbitration administered by [Arbitration Provider], except that either party may seek injunctive relief in court for intellectual property or confidentiality violations. You waive any right to participate in a class action lawsuit or class-wide arbitration.

These Terms are governed by the laws of the State of [State], without regard to conflict of law principles.

12. Termination

Meridian may suspend or terminate your access to the Services at any time for violation of these Terms, for failure to pay applicable fees, or for any reason with reasonable notice. You may cancel your membership at any time as described in our Cancellation Policy. Termination does not relieve you of payment obligations for the current billing period.

Important Safety Information (ISI) — GLP-1 Receptor Agonists

GLP-1 receptor agonists (semaglutide, tirzepatide, liraglutide) are FDA-approved prescription medications. They are not appropriate for everyone. Do not use if you or a family member have a history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).

Serious risks include: pancreatitis, gallbladder problems, kidney problems, hypoglycemia (in patients using insulin or insulin secretagogues), heart rate increase, suicidal ideation and behavior, and serious allergic reactions. Tell your provider about all medications you take and all medical conditions.

The most common side effects are nausea, diarrhea, vomiting, constipation, and stomach pain. These are usually mild and decrease over time.

Not for use in patients with type 1 diabetes. If you experience severe abdominal pain, stop the medication and seek immediate medical attention. For full prescribing information, consult your Meridian Health provider.
